How to refer a client to a wheelchair and seating and postural management outreach clinic.
Points to note
Your documentation must be complete and with our coordinator at least 2 weeks before the advertised clinic date
We do not accept late requests unless there is a genuine and exceptional circumstance for a late submission
There is a limit to the number of clients we can see in a clinic: we do not overbook clinics.
Making a referral
Submit your request through ProWorkflow (PWF), along with the relevant documents:
