This checklist is a tool for EMS Assessors to use when considering applications for housing modifications funded by Whaikaha – Ministry of Disabled People through it's two contracted providers, Accessable and Enable New Zealand. It is designed as a supplement to prompt critical thinking around the person’s eligibility and essential need and can be used prior to submitting an application.
